For enrollment in all programs except Barn Babies, we will need the following:
- MO 500-3312 Enrollment Form — Fillable PDF you can submit online to us
- Liability Waiver — included with the above form
- Photo Release (Yes or No) — included with the above form
- MO 500-3303 Child Medical Examination — Requires signature by doctor and email submission to us
- A current copy of child's vaccination records — and/or —
- MO 580-1723 Vaccination Exemption Form
🩺 A note about vaccinations
We get a lot of questions about vaccinations! In concert with State regulations, we are required to notify all families that under the State vaccination exemption policy, we are allowed to have, and do have, un-vaccinated children attending our school.
Under Title XI, Section 167.181.3 — This section shall not apply to any child if one parent or guardian objects in writing to his school administrator against the immunization of the child, because of religious beliefs or medical contraindications. In cases where any such objection is for reasons of medical contraindications, a statement from a duly licensed physician must also be provided to the school administrator.
We absolutely recommend Tetanus or DTaP (Diphtheria, Tetanus, Pertussis) vaccination for all children attending.
