For enrollment in Farmschool and Farmer Friday, we will need all of the below. For Homeschool Enrichment, we will only need the MO 500-3312 Enrollment Form.

MO 500-3312 Child Care Enrollment Form — This is an online form that will automatically be sent once completed. There is no need to provide a paper copy. If you have previously completed a paper copy, please still submit this again. Thank you!
MO 500-3303 Child Medical Examination — Requires signature by doctor and email submission to us
We do accept non-immunized children with appropriate exemption form

Please provide a copy of immunization records (see link for more details about Missouri immunization requirements). We accept Religious Immunization Exemption forms which can be found at the link below. This appears to be a changing requirement in Missouri, so please be prepared for changes.

Immunization Requirements | Religious Immunization Exemption

Immunization Exemption: Under Title XI, Section 167.181.3 — This section shall not apply to any child if one parent or guardian objects in writing to his school administrator against the immunization of the child, because of religious beliefs or medical contraindications.
